Making an Appointment
1. Please call 209-228-2273 for an appointment or book online on the My Health Portal.
2. Please arrive 10-15 minutes before your appointment. Sometimes you may need to give a urine sample, do other testing, or fill out a questionnaire.
3. Please bring all records and a list of medications.
