Student Health Services, like many UCs and other college health centers, does NOT provide sick notes or verification of illness forms to students requesting a medical excuse for class, assignments or activities.
Providers do not provide retroactive sick notes or provide notes for professors to delay exams, tests, or assignment due dates. We cannot provide proof of your minor illnesses to your professor. It is not possible to give retrospective confirmation for common medical conditions like headaches, colds, diarrhea, nausea, etc.
Please avoid making appointments at Student Health Services to secure such a note or a verification of visit. In the past, students would come for an appointment just to provide this documentation. This reduces the number of available appointments for students who have more pressing health care needs and symptoms.
Please notify your professor if you are unable to attend a class or miss an assignment- preferably before the assignment is due or before you miss the class. It is a good idea to familiarize yourself with class attendance policies and missed assignment guidelines.
