IMPORTANT NOTICE REGARDING UC SHIP & YOUR SOCIAL SECURITY NUMBER

All students who enroll in UC SHIP must provide their social security number to their campus registrars’ office in order to be sent the federal 1095-B form for their tax records. UC SHIP’s enrollment vendor, USI, will make 3 attempts per year via postal mail if your social security number is not on file with your campus registrars’ office.
