UCSHIP WAIVER
- To qualify to waive, the student must have a primary care provider within 55 miles, especially for students with Medi-Cal
- A copy of the front and back of their insurance card must be submitted with the application; a summary of benefits, if available
Fall Waiver Period: April 28, 2025 to July 28, 2025 @ 11:59 PM
- A new waiver is needed every academic year
- Insurance Services is available to assist. Contact them via:
- Office Hours: Mon–Fri 1:30–4:30 PM on the 2nd floor of the Student Health Center across from the reception desk
