Immunization Requirements

The deadline for Fall 2017 is Wednesday, August 23.

Please follow the instructions to complete your requirement.

Below are details on how to comply with the UC Immunization Requirement.

All new students must also complete a TB Risk Screening. For background on this requirement, see the University of California Immunization Plan.

Required Immunizations

 

Vaccine Proof of Immunization Required
Measles, Mumps and Rubella (MMR) Two doses; first dose on or after age one - OR - Titer (blood test) showing immunity.
Varicella (chickenpox) Two doses; first dose on or after age one - OR - Titer (blood test) showing immunity.
Tetanus, Diphtheria and Pertussis (Tdap) One dose on or after age 7 (note: Td/DTaP do NOT satisfy the requirement).
Meningococcal Conjugate (Serogroups A, C, Y, & W-135) One dose on or after age 16 for all students that are under 22 years of age (students 22 or older may leave blank).

  1. Prepare for online submission:
    1. Review the step-by-step instructions
    2. Complete the Immunization Worksheet
  2. Log into the myHealth patient portal and open the immunization form. 
  3. Fill out the relevant dates for required immunizations. 
  4. Add image or PDF file of immunization records. 
  5. Submit the form online.
